Donors – Screening Questionnaire Details- Stakeholder Meeting – 04/19/2022

The Screening Questionnaire Detail Processing screen will allow users to capture responses to interview questions, with conditional formatting to draw attention to unexpected responses that result in deferrals and/or impact donor eligibility.  The interview will allow for the configuration of multi-level follow-up questions, as well as questions that allow for multiple responses, such as medications.

Stakeholder Meeting Notes – 04/19/2022

• Consider allowing comment entry for certain questions such as OK per PCP
• For health histories allow more detail in date capture, such as first/last dose for greater flexibility on deferral creation
• Include/Display question and/or questionnaire versions
• Consider methods for confirming previously documented health history while still requiring responses to questions as regulated (i.e. pre-populate or pull forward previous medication history).
